This number compares with only 19 such errors in 1987 (1). All errors were averted before drug dispensing from the pharmacy.The drugs involved were diltiazem (32 errors), nifedipine (27), verapamil (23), theophylline (14), propranolol (5), morphine (4), procainamide (4), isosorbide (3), albuterol (3), divalproex (2), and disopyramide (1).
