Abstract
An overview is presented on spinal disorders in the community based on Swedish statistics gathered over the last 15 years and projected for the year 2000. Commonly encountered spinal problems are discussed, and their need for both nonsurgical and surgical treatment by the year 2000 is calculated.
Keywords
Medicine
Spine1987 Volvo Award in Clinical Sciences: A New Clinical Model for the Treatment of Low-Back Pain
1,240 Citations1987Gordon Waddell
Observations of natural history and epidemiology suggest that low-back pain should be a benign, self-limiting condition, that low back-disability as opposed to pain is a relatively recent Western epidemic, and that the role of medicine in that epidemic must be critically examined.
SpineLong-term Follow-up of Lower Lumbar Fusion Patients
471 Citations1987Thomas R. Lehmann, Kevin F. Spratt +5 more
Although patients after lower lumbar fusion report more pain than the general population and have more radiographic evidence of instability and stenosis than expected for the generalpopulation, they are generally doing well and are satisfied with the results of their surgery.
Acta Orthopaedica ScandinavicaThe Lumbar Disc Herniation: A Computer-Aided Analysis of 2,504 Operations
440 Citations1972Erik Spangfort
SpineAssessment of Severity in Low-Back Disorders
337 Citations1984Gordon Waddell, Chris J. Main
Physical Impairment and disability are defined, simple clinical methods of assessing them in low-back disorders are described, and the relationship between them analyzed.
SpineA Comparison of Radiographic Findings in Fusion and Nonfusion Patients Ten or More Years Following Lumbar Disc Surgery
247 Citations1979John W. Frymoyer, Edward N. Hanley +3 more
Although complex radiographic changes follow lumbar disc surgery, with or without failure, it is concluded that the plane radiograph is of little aid in determining the source of postoperative pain.
SpineRelationship between Traumatic Spinal Canal Stenosis and Neurologic Deficits in Thoracolumbar Burst Fractures
209 Citations1988Tomoyuki Hashimoto, Kiyoshi Kaneda +1 more
The stenotic ratios of the area occupied by the retropulsed bony fragments to the estimated area of the original spinal canal are calculated and the shape of the narrowed canal and the disruption of spinal elements are examined.
Acta Orthopaedica ScandinavicaAn historical perspective on low back pain and disability
194 Citations1989David Allan, Gordon Waddell
This review of low back pain and sciatica over the past 3500 years tries to put the present epidemic of lowBackache into historical perspective and concludes that much low back disability is iatrogenic.
Journal of Pediatric OrthopaedicsLong-Term Clinical and Radiological Follow-Up of Spondylolysis and Spondylolisthesis
166 Citations1987Helena Saraste
A clinical and radiological follow-up study with at least 20 years of observation was made of 255 spondylolysis and spondyolisthesis patients for examination of the clinical course and its possible correlation to radiographic findings.
Clinical Orthopaedics and Related ResearchAdvances in Low-Back Pain
155 Citations1985Alf Nachemson
guided activation of large muscle groups is a benefit for the structures of the back as well as the body's own pain-reducing enkephalins, which should be avoided.
SpineThe Neurological Outcome following Surgery for Spinal Fractures
132 Citations1988S. D. Gertzbein, C. M. Court-Brown +5 more
Positive correlations were found between the level of injury and Frankel grades, and the cord lesions tended to demonstrate more severe neurological deficit, whereas the cauda equina lesions were associated with a lesser severity of neurologic deficit.
Journal of Bone and Joint SurgeryThe randomized clinical trial in orthopaedics
132 Citations1985Sally Rudicel, John M. Esdaile
A recently proposed variation of the classic randomized clinical-trial model, the randomized-surgeon design, appears to solve some of the problems of theclassic randomized clinical trial.
Journal of Bone and Joint Surgery - British VolumeThe long-term results of fusion in situ for severe spondylolisthesis
124 Citations1983Johnson, EO Kirwan
It is concluded that although fusion in situ does little to improve cosmesis very few patients complain about their appearance and that it is a safe and reliable method of treatment for severe spondylolisthesis which has stood the test of time.
Journal of Bone and Joint SurgeryThe changing pattern of scoliosis treatment due to effective screening.
108 Citations1981Gunnar Torell, Anders Nordwall +1 more
SpineDecompression and Circumferential Stabilization of Unstable Spinal Fractures
93 Citations1988S. D. Gertzbein, C. M. Court-Brown +7 more
The authors conclude that this technique is an effective and safe method for treating unstable thoracolumbar injuries and is recommended if anterior instrumentation is unavailable.
Journal of Bone and Joint SurgeryResults of surgical treatment of adults with idiopathic scoliosis.
93 Citations1987Paul D. Sponseller, Melissa Cohen +3 more
The outcome of surgical treatment of idiopathic scoliosis in forty-five adults was studied with special attention to pain, function, self-image, and pulmonary function, and the high rate of complications.
SpineLong-Term Anatomic and Functional Changes in Patients with Adolescent Idiopathic Scoliosis Treated with the Milwaukee Brace
82 Citations1985Thomas Cochran, Alf Nachemson
Those patients who complete a Milwaukee brace treatment for adolescent idiopathic scoliosis from a functional and social point of view do very well, demonstrating that the effectiveness of the Milwaukee brace is demonstrated.
Clinical Orthopaedics and Related ResearchMobility in the Lumbosacral Spine After Fusion Studied with the Aid of Roentgen Stereophotogrammetry
62 Citations1977T. H. Olsson, Göran Selvik +1 more
Twelve cases operated upon with fusion in the lumbosacral spine were studied with a roentgen stereophotogrammetric method and it was found that the possible stabilization of a fusion takes place within 180 days.
PainWhen back pain becomes disabling: a regional analysis
57 Citations1988Ernest Volinn, Daniel Lai +2 more
Investigative RadiologyKinematic Analysis of Spinal Fusions
24 Citations1976T. H. Olsson, G. Selvik +1 more
Two cases with clinical suspicion of incompletely healed anterior fusion between L5 and S1 were examined by means of a roentgen stereophotogrammetric method and a pseudarthrosis was found.
Journal of Pediatric OrthopaedicsThoracic Adolescent Idiopathic Scoliosis
14 Citations1987Michael D. Ryan, Alf Nachemson
A perinatal and environmental survey was done on 551 adolescent patients with idiopathic thoracic scoliosis to determine if maternal age, paternal age, patient's sex, birth order, season of birth, birth weight, weight at 10 years, socioeconomic group, or population density influenced the severity of the curve.
SpineTransfusion-Related Non-A, Non-B Hepatitis in Elective Spine Deformity Surgery Patients in Gothenburg, Sweden
7 Citations1989P. J. Landin, Alf Nachemson
The incidence of non-A, non-B hepatitis in patients after receiving homologous blood transfusion for elective spine deformity surgery, at Sahlgren Hospital was low when compared with an overall incidence of 4 to 15%, as presented in several large prospective studies.
