TL;DR

This study examined the hypothesis that previous evidence of renal disease would independently predict future cardiovascular disease and whether ramipril continued to be effective in patients with impaired renal function, and whether the common clinical practice of withholding angiotensin-converting enzyme (ACE) inhibitors in patientsWith impaired renal excretory function is justified.

Abstract

In patients who had preexisting vascular disease or diabetes combined with an additional cardiovascular risk factor, mild renal insufficiency significantly increased the risk for subsequent cardiovascular events. Ramipril reduced cardiovascular risk without increasing adverse effects.
