Abstract
Complete serial section study of the thrombosed segments of the coronary arteries from 20 consecutive cases of fatal coronary thrombosis revealed that in each instance the thrombus was anchored in fissures of the surrounding atheromatous plaques. Most of the accompanying plaque hemorrhages could be traced to an entry of blood from the lumen through the same or other cracks in the atherosclerotic artery wall, although some of them also resulted from the rupture of plaque capillaries. There were no cracks in the wall of 16 atherosclerotic coronaries without thrombosis that were studied in the same manner. These findings suggest that the sudden break of the brittle lining of atherosclerotic coronary arteries represents a major factor in the causation of human coronary thrombosis.
