PMCF (Post Market Clinical Follow Up)
Definition
Post Market Clinical Follow up (PMCF) is the systematic and ongoing process of collecting and evaluating clinical data on a medical device after it has been placed on the market and is in routine clinical use. Under the European Union Medical Device Regulation (EU MDR 2017/745), manufacturers must address PMCF in their post market surveillance plan, either with a PMCF plan or with a justification of why PMCF is not applicable, and it is part of the broader post market surveillance system for medical devices.
Why it matters
Medical devices enter the market through a conformity assessment process that relies on clinical evidence available when the device is first placed on the market. However, this premarket evidence is often limited in scope: clinical investigations may involve relatively small numbers of patients, short follow up periods, and controlled conditions that do not fully reflect real world clinical use. As a device is used by a broader range of patients, in diverse clinical settings, and over longer time periods, new information about its safety, performance, and clinical benefits may emerge. PMCF addresses this by creating a structured process for continuing to collect and evaluate clinical data after market placement. Under the EU MDR, the manufacturer must address PMCF for devices of every class: the post market surveillance plan contains either a PMCF plan or a justification of why PMCF is not applicable (Annex III), and PMCF is usually most extensive for higher risk devices such as Class III and implantable devices. The findings are documented in a PMCF evaluation report that forms part of the clinical evaluation report and the technical documentation (Annex XIV, Part B).
How it works
The PMCF process begins with a PMCF plan that defines the objectives, methods, and timeline for post market data collection. The plan is part of the post market surveillance plan and is linked to the clinical evaluation plan, creating an integrated framework for ongoing evidence generation.
PMCF activities may include dedicated PMCF studies (prospective clinical investigations designed to answer specific clinical questions about the device's performance and safety in post market use), patient registries that systematically collect data on device recipients, surveys and questionnaires completed by clinicians or patients, analysis of complaint and vigilance data, review of published literature on the device or equivalent devices, and analysis of real world data from electronic health records or insurance databases.
The scope and intensity of PMCF activities are proportionate to the risk class of the device and the maturity of the existing clinical evidence. Novel, high risk devices (such as implantable cardiac devices, total joint replacements, or surgical mesh) require more extensive PMCF than well established, lower risk devices with decades of clinical experience. The PMCF plan must be updated as new evidence accumulates and as questions about the device's clinical performance evolve.
PMCF evaluation reports summarize the findings from PMCF activities and assess their implications for the device's ongoing safety and performance. These reports feed into the clinical evaluation report, which the EU MDR requires to be updated throughout the device's life cycle with PMCF data; for Class III and implantable devices, the PMCF evaluation report and, if indicated, the summary of safety and clinical performance must be updated at least annually (Article 61(11)). If PMCF data reveal new safety concerns or changes in the benefit risk profile, the manufacturer must take corrective actions, which may include design modifications, labeling changes, field safety corrective actions, or, in extreme cases, withdrawal of the device from the market.
The EU MDR has substantially strengthened PMCF requirements compared to the predecessor Medical Devices Directive. PMCF must now be addressed for all device classes (with a PMCF plan, or a justification of why PMCF is not applicable), timelines and methods must be specified in the PMCF plan, and notified bodies scrutinize PMCF activities during conformity assessment audits. The Medical Device Coordination Group (MDCG) has published templates for the PMCF plan (MDCG 2020-7) and the PMCF evaluation report (MDCG 2020-8).
When to use it
Address PMCF for every medical device placed on the EU market under the MDR, normally with a PMCF plan whose scope is tailored to the device's risk class, novelty, and the gaps identified in the premarket clinical evaluation; where PMCF is not considered applicable, the post market surveillance plan must justify why. Begin PMCF activities upon market placement and continue them throughout the device's lifecycle. Review and update the PMCF plan regularly based on emerging evidence, post market surveillance data, and regulatory guidance.
Key takeaways
- Medical devices enter the market through a conformity assessment process that relies on clinical evidence available when the device is first placed on the market.
- The plan is part of the post market surveillance plan and is linked to the clinical evaluation plan, creating an integrated framework for ongoing evidence generation.
- Address PMCF for every medical device placed on the EU market under the MDR, normally with a PMCF plan whose scope is tailored to the device's risk class, novelty, and the gaps identified in the premarket clinical evaluation; where PMCF is not considered applicable, the post market surveillance plan must justify why.
- A common mistake is treating PMCF as a one time activity rather than an ongoing process.
Common mistakes
A common mistake is treating PMCF as a one time activity rather than an ongoing process. The EU MDR requires that PMCF continue throughout the device's lifecycle, with the plan updated as questions evolve and evidence accumulates. Another error is relying exclusively on published literature review without conducting proactive data collection, particularly for devices where published data are scarce or insufficient to address device specific questions. Designing PMCF studies that are too small or too short to answer the clinical questions they are intended to address is a further limitation. Some manufacturers conflate PMCF with general post market surveillance (which includes nonclinical activities such as complaint handling and trend reporting) without recognizing that PMCF specifically concerns clinical data and clinical evaluation. Finally, failing to integrate PMCF findings into the clinical evaluation report creates a gap between available evidence and regulatory documentation.
Example
A manufacturer of a new bioresorbable coronary stent develops a PMCF plan under the EU MDR. The plan includes a prospective, multicenter PMCF registry enrolling 2,000 patients at 40 hospitals across Europe, with follow up visits at 6, 12, 24, and 60 months. The primary objective is to confirm that the rate of target lesion failure at 12 months in routine clinical use is consistent with the premarket clinical investigation results. Secondary objectives include evaluating performance in patient subgroups not well represented in the premarket study (patients over 80, those with calcified lesions, and those with diabetes) and monitoring for very late scaffold thrombosis beyond 24 months as the polymer resorbs. Annual PMCF evaluation reports summarize the accumulating data and feed into the clinical evaluation report reviewed by the notified body.
